Middle School Volleyball and Football Deadline

The following is required for any student to participate in athletics at SCMS, All items must be completed on the www.registermyathlete.com website:

• A new physical exam each year on the approved 2023-2024 A.I.A. form 
• Athletic Handbook (inlcudes concussion requirements per CJHL & AIA Bylaws)
• Submit Medical Insurance Card/Coverage (mandatory) on Regtister My Athlete
• Permission for BinaxNOW Covid-19 testing for students
• Provide proof of being fully vaccinated against Covid-19 (immunizations or card) 
  • Medical Insurance is Mandatory for all students to provide
• Athletic Fee for the 2023-2023 school year

Must be Paid on ww.registermyathlete.com site

Middle School Fee $25 per student

All fees must be paid online $2.50 will be included.
